A questionnaire is a short survey for your clients that is attached to a service.
If a client books a service with a questionnaire attached, they will be prompted to fill it out after their booking has been confirmed or is pending and awaiting confirmation.
Any new client questionnaire response will be tagged in the client list.

A log of all questionnaire responses can be accessed on a client's overview page so you have a record of all submissions and can meet client preferences and requests efficiently.

If your client has submitted a response but needs to re-submit, click the Resend Questionnaire button in the upper right corner.

There is a Resend Questionnaire notification already set up if you wish to prompt clients to submit a response. To send a client notification, you can read this article.
For mobile
On mobile, you can access client questionnaires by clicking Menu > Clients.
Then, select a client and click the Questionnaires tab.
